The referral should also include the reason for the referral and any other pertinent information such as the patient’s insurance information.

The nurse practitioner’s responsibility for follow-up after the referral is to ensure that the patient has scheduled and attended the consultation with the specialist. The nurse practitioner should also follow up with the patient after the consultation to discuss the specialist’s recommendations and any further management or treatment plans.
